Describe the task and environment
Record app version, build, distribution channel, device model and operating system. Add relevant account state, language and network conditions without exposing credentials. Distinguish a new account from an existing one with saved data. The same screen can behave differently after a restore, permission denial or interrupted migration, so record those conditions when they affect the observation.
Separate expected and actual outcomes
Write the steps in order and identify the first unexpected result. Say how often it happens and whether a restart changes it. Avoid diagnosing the cause unless you have evidence; ‘the server is broken’ is less useful than the visible error and time. Keep logs redacted and screenshots free of personal data, access tokens, card information and unrelated notifications.
Connect the report to a retest
Assign an owner to reproduce the issue and ask focused follow-up questions. When a fix ships, repeat the original sequence on the identified build and device category. Add the result to the same record rather than deleting the report. Group duplicates by underlying conditions, while keeping observations that conflict; a single successful retry may not resolve an intermittent failure.

Example steps should be specific enough that a colleague can attempt them without asking what ‘do the normal thing’ means. Leave unknown fields marked unknown instead of inventing a device or cause.
