Group the underlying condition rather than keywords

Two users mentioning ‘login’ may describe different failures, while several comments about lost work may share one restore defect. Record task, build, device and preconditions. Try to reproduce significant issues before assigning a cause. Keep contradictory observations visible, because a problem that appears only on one account state can disappear during a clean-install test.

Choose priority from impact and confidence

Blocked core journeys, data loss and incorrect payment access usually need investigation before minor visual preferences. Assess how many relevant reports show the issue and how confidently the team understands it. Do not treat a small cohort’s frequency as a population estimate. Give uncertain but severe issues an investigation task rather than forcing every finding into a misleading numeric score.

Close the loop with a specific retest

Assign an owner, expected evidence and the build containing the change. Ask affected participants to repeat the original steps and use fresh users when the fix changes comprehension. Mark a finding resolved only within the scope tested. Keep remaining limitations in the release record and acknowledge useful criticism. A shipped patch without a retest should be labeled shipped, not proven fixed.
